The purpose of this brace is to keep the lower and upper leg in proper alignment. Because I am tall and have long legs, when I am driving or at the computer I tend to let my bent right leg splay outward, which puts diagonal pressure on the knee joint. When this is done for long periods, the knee becomes too flexible and prone to slip out of place when I'm walking, causing the leg to collapse and sending me sprawling to the ground (which has happened twice in public). Also, once, after remaining at the wheel all day on a long trip, I had a sore knee and a limp for two months.
This appliance's hinged braces on both sides of the knee are all that are needed to keep my knee properly aligned, so there is no need to adjust it too tightly. The bottom strap is only to keep the Velcro closure from coming undone; and the top strap should be adjusted so that the brace is simply snug. If applied too tightly, leg circulation will be impeded which can result in numbness and pain. If worn this way, the knee joint will not fail, the brace is easily adjusted to, and leg movement is not impeded.

With a damaged meniscus, I needed pressure and support directly around the kneecap. This brace did not provide that. No matter how tight the straps are fastened, the area around the kneecap remains loose since the back is open. The brace is well-made. But this was the wrong brace for my problem.

Knee replacement is coming--no lateral stability and lots of pain. This is best of the 3 I've used, and cheapest. I have two wide velcro straps (detached, not flexible) for a former brace.
When I noticed the neoprene on this one beginning to stretch (its best feature), I started just snugging the brace fastenings--firmly below the knee, it's harder to reach if you're older, arthritic and overweight.
I then apply the loose straps over the brace straps, and cinch them as tightly as I require. I'm overweight and that soft fat is compressible without interfering with circulation, but I wear the brace only 4 hours at a time.
Best of both--I'm not destroying the neoprene that makes this brace comfortable, easy care, overall very useful. Those add-on velcro straps provide the just plain hard pull that my plump legs require.

I'm ducking knee surgery. This is the best hinged brace under $50 that I've tried. Since I wear it often, it began to stretch a bit. So, I took the loose velcro straps off a failed brace, and added them to the outside of this brace for extra tightness as needed. The brace is bulky for many women, but all hinged braces have to increase the inside of the knee; all braces with built-in straps may stretch. The failed brace fell apart at the hinges' vertical stitching, and that can't happen to this brace. Overall: a standout.
