Professional test: For the professional test we advise that the completion of the Custody and Control Form (CCF) and collection of the urine sample is carried out by trained personnel. Please also ensure to read all instructions before proceeding. A test will be voided if the CCF is not completed correctly and/or the sample is not taken as per the instructions.
Collector ensure the Specimen ID number on the top of the CCF matches the Specimen ID number on the label
Collector identifies the donor by photo ID. Complete Step 1 (collection site information, donor site information and reason for test) on the CCF
Important: Check the Professional Test (4000) test box
Collector gives a collection container to the donor for providing a specimen
Donor watches collector pouring the specimen from the collection container into the specimen bottles, placing the cap on the specimen bottles, and affixing the label/seal on the specimen bottle(s)
Collector dates the specimen bottle label after they are placed on the specimen bottle
Donor initials the specimen bottle label after it is placed on the specimen bottle
Complete any additional information requested on Step 4 and 5
Fax the white copy of the CCF form to the Medical review officer at - 321-872-0083. Then place the sealed collection device into the pouch of the clear transport bag containing the absorbent pad. Place the white copy of the CCF into the other pouch of the transport bag and seal the bag. Place the transport bag into the white plastic envelope for extra protection, then place into the shipping box provided to you. Fill necessary information on the FedEx label and apply label to the FedEx shipping bag. Ship immediately
Confidential/anonymous test: Performing a confidential test means that you are not required to submit any personal information on the Custody Control Form (CCF) about the donor. This does not affect the quality of the laboratory analysis or results; however, you will not receive a Medical Review Officer (MRO) report.
Enter N/A on organization name. Enter anonymous (or leave blank) on donor name, Leave donor ID number blank. Check "other" box for test reason
Important: Check the Anonymous Test (4226) test box
Deposit urine into collection container. Pour urine in specimen container into the specimen bottle and seal bottle
Date the specimen bottle label and place on the specimen bottle
Complete any additional information requested on Step 4 and 5
Then place the sealed collection device into the pouch of the clear transport bag containing the absorbent pad. Place the white copy of the CCF into the other pouch of the transport bag and seal the bag.
